Fill out the above order form completely. Enclose check or money order for total amount (plus tax if applicable). Orders may also be charged to your MasterCard/Visa/Discover. Orders are processed within 24 hours upon receipt. |

| Sales Tax |
Kansas residents only must add local sales tax rate unless you provide a copy of tax exemption certificate. |

| Shipping Information |
All items are shipped ground service U.P.S. at customer's expense. Air service is available to all customers at customer expense (call for charges). All shipments outside the continental U.S.A. will be sent Post Office or U.P.S at customer's expense. |

| Damaged Merchandise |
All shipments are insured. Please inspect merchandise IMMEDIATELY. Should there be loss or damage, it is the buyers responsibility to file a damage claim with the carrier within 15 days of receipt of merchandise. |

| Returns |
If, for any reason, you are dissatisfied with any item, you may return it to us. BEFORE returning, you must contact us and explain the reason for returning. We will then issue a "Return Authorization".
Returns may be subject to a 15% restocking fee. The customer is responsible for the freight paid for the original shipment to the customer and the return freight to Truth on Tape. A note of explanation should accompany the parcel along with a means of contact if a question arises. Please ship all packages to our warehouse. |

| Returned Checks |
If checks are returned by your bank for ANY reason, there will be a $15.00 handling charge. If the matter is not handled immediately, the information will be turned over to the appropriate District Attorney's office. |
